When combined with a high deductible health insurance plan, possibly funded through your Health Savings Account (HSA) or Flex Spending Account (FSA), the PrimeCARE program’s pre-paid prevention and wellness option offers significant savings, convenience and simplicity in meeting your health care needs. 

For a yearly subscription fee, the following benefits are provided at no extra cost.

  • Initial consultation and health risk assessment
  • Routine primary care visits at participating PrimeCARE offices (excluding obstetrics)
  • Routine prevention and wellness lab and x-ray (as specified in contract), pap smear, EKG, pulmonary functions, vision and hearing screening (only if performed in PrimeCARE offices)
  • Routine adult flu, tetanus and pneumonia immunizations (immunizations under age 2 available at cost)
  • DEXA scan every 2 to 3 years if appropriate
  • Nutrition, diabetes and exercise counseling
  • Skin cancer screening and removal of suspicious lesions (pathology charge not included)
  • Urgent in-office care of minor lacerations and injuries
  • Coordination of care with hospitalization and referral to specialists
  • After-hours access to PrimeCARE team through physicians exchange
  • Interactive website at www.capstone.md for timely information and updates

PlusCARE

This "full-service" program is the perfect complement to your existing health insurance plan. With your membership you have 24/7/365 direct access to your CapstoneMD physician...and much more:

  • Initial consultation and comprehensive health risk assessment
  • 24 hours per day/7 days per week/365 days per year access to the doctors and/or the team via telephone, text, or email for non-urgent communications
  • Same business day or next business day office appointments (Saturday and Sunday appointments available)
  • Personalized coordination of insurance, HSA and FSA benefits as they apply to the clinical in-office services given by CapstoneMD team
  • Little or no time in the office waiting room
  • Personalized and prioritized referrals to specialists as necessary (fees paid to specialists not covered under this agreement)
  • Portable personal electronic medical record
  • Phone or online consultation via e-mail at no additional charge
  • House calls (within 15 mile radius of CapstoneMD office) as deemed necessary or appropriate by the team (additional fees may apply)
  • Priority management of hospitalization at hospital of patient’s choice
  • Interactive www.capstone.md website for timely information and updates
  • CapstoneMD PlusCARE card for quick reference and identification

TotalCARE

This encompassing program greatly simplifies your primary prevention and wellness care by offering a fixed monthly membership rate that combines  all the benefits of PrimeCARE with those of PlusCARE.  It offers the convenience of easy access to your CapstoneMD physician 24/7/365 along with several other benefits that will greatly enhance your health care. You may want to save on your total insurance costs by purchasing a high-deductible insurance and utilize an HSA option (Health Savings Account).

  • 24 hours per day/7 days per week/365 days per year access to the doctors and/or the team via telephone, text, or email for non-urgent communications
  • Same business day or next business day office appointments (Saturday and Sunday appointments available)

  • Personalized and prioritized referrals to specialists as necessary (fees paid to specialists not covered under this agreement)
  • Secure online access to personal medical records by CapstoneMD physicians

In addition to all the Access services, the following clinical services, delivered at the CapstoneMD's office (collectively, the “PrimeCARE services”) will also be covered by the annual pre-paid fee:

If you choose to become a member, simply sign up online. Soon after enrollment, schedule a comprehensive health assessment visit and get your personal wellness plan rolling!

SERVICES NOT COVERED:

  • Services performed by practitioners or specialists other than CapstoneMD physicians and staff
  • Ongoing clinical services such as behavioral counseling, physical therapy, occupational therapy, speech therapy, etc.
  • Any services related to pregnancy or infertility
  • Prescription medications or chemotherapy
  • Medical devices
  • Hospitalization 
  • Cosmetic or aesthetic services (discounts available for selected Botox/Dysport procedures)
  • Emergency response, diagnosis, transport or treatment


Clinical Services are limited ONLY to those which are provided in CapstoneMD’s offices and as listed above. It is again strongly suggested that these plans be used along with and not in place of a standard Health Insurance Plan.  THIS IS NOT AN INSURANCE PLAN. This only covers limited in-office care at CMD offices as outlined above.
